To receive service from a PMA licensed practitioner you must first complete the Informed Consent and Request for Admission and Service form (ICRAS). If you have already submitted the form on a previous visit, please click “No” to proceed to scheduling.
To expedite your first visit with the PMA practitioner please click “Yes” and proceed to the form. Your completed form will be sent through the PMA secure server and emailed directly to the practitioner. If you have any questions, please contact me at info@loveofearth.com.
You will need the following information complete the ICRAS form.

Notice: Pastoral Services do not include the diagnosis or treatment of emergencies or disease. If you are experiencing emergencies, trauma, advanced disease or symptoms that are unknown or that potentially could be a disease condition, you should always consult a conventional health care practitioner or seek emergency services.
INSURANCE COVERAGE: Many U.S. State employee insurance plans, and private insurance companies in the U.S. and Canada pay for Pastoral Health Services. Your services should also be deductible on your U.S. income tax return. Most Pastoral Practitioners do not file with your insurance, but will provide you with a copy of your receipt for filing.
